Often the transition to a skilled nursing and rehabilitation facility is challenging. Premier Rehabilitation

A stay at Yuba City Post-Acute begins with an evaluation by our team of physicians, therapists, nurses, social services, and other support staff. These meetings allow us to design a personalized care plan focused on returning residents to their highest level of independence as quickly as possible.

We offer a unique and personalized experience for residents recovering from strokes, neurological impairments, and orthopedic procedures. Rehabilitation takes place in our therapy gym under the guidance of our experienced professional therapists.

• All rooms have new electric high/low beds, and cable TV (private phones are also provided for our short-term rehab units).
• The full in-house rehab unit provides: PT, OT, ST.
